Ifeoma Okolo is the Chief Executive Officer of Concord Primary Care Services, Inc., and is also the Director of Clinical Services.
Ifeoma's nursing career spans 30-years. She has held positions as Director of Nursing, Field Supervisor, Field Nurse, and Nurse in the States of The District of Columbia; Maryland; and Texas, USA. She is a licensed Nurse in the State of Texas.
Ifeoma earned a Diploma in Nursing from University College Hospital, Ibadan, Nigeria; a Bachelor of Arts in History from The University of Lagos, Nigeria; and a Master of Arts in Political Economics from the Institute of Social Studies, The Hague, Netherlands.
Sara Perez
Administrative Assistant

With a certificate in Front Office Administration from San Antonio College, Sara joined Concord Primary Care Services, Inc. in 2020 as Administrative Assistant. Sara is bilingual in English and Spanish languages, and her administrative support duties include office calendar management, preparing reports both internal and for government regulatory agencies.
Sara’s excellent interpersonal skills, Internet research skills and project coordination experience make her a valuable member of Concord’s compassionate staff who works very well with clients.
Jennifer Okolo
Senior Operations Manager

As the Senior Operations Manager of Concord Primary Care Services, Inc., Jennifer Okolo is responsible for the daily cost administration and management of our organization. She also supervises our State Program Coordinators, and is responsible for reviewing and approving all insurance billings.
Jennifer Okolo is a marketing graduate of the Institute of Management and Technology, Enugu, Nigeria, and has been employed at Concord Primary Care Services, Inc. since 2000.
Maria Valdez

Maria joined Concord Primary Care Services, Inc. in 2020 as an Electronic Visit Verification (EVV) Liaison.
Maria works closely with clients, patients and service providers to ensure high quality care delivery in a timely manner. Maria ensures that accurate information is provided to the billing department for effective financial management.
Marie Zamot
Marie Zamot joined Concord Primary Care Services in 2010 as our State Program Liaison and Field Staff Liaison.
Marie Zamot attended NWV College, Texas to attain her Associates Degree in Science, and has received an Instructional Certification from the Texas Education Agency.
In her daily role at Concord, Marie Zamot establishes attendance to client partnerships, conducts client admissions, and is in association with numerous insurance company case managers. Among several attributes, Marie Zamot is proficient in marketing strategies and concepts, and all internal office operations.
Belle Neve, MA
Belle Neve began working in Concord Primary Care Services in 2001 as an Executive Assistant. She has spearheaded implementation of new programs and projects in Concord as a Cross-Functional Consultant in Human Resources, Marketing, Public Relations, and Operations before becoming our Community Liaison.
Belle earned a Bachelor of Dance and History of Art degree from Hamilton College, New York. She completed a Bachelor of Arts Special Program in Acting, Ceramics, Choreography, Film, and Musical Theatre from New York University Gallatin, Tisch, and Steinhardt School of Culture, Education, and Human Development. Belle also completed a Master of Arts degree in Journalism with Media Law from the University of Arts, London College of Communication.
Raymond Moreno, BSN, RN

Raymond Moreno began working at Concord Primary Care Services in 2015 as the CLASS Program Liaison and Supervisory Registered Nurse.
Raymond Moreno, BSN, RN, has a Bachelor of Science in Nursing degree from The University of the Incarnate Word, Texas, and has over 25 years of nursing and supervisory experience. Nursing experience is throughout the adolescent to adult and geriatric lifespan. Raymond Moreno has held positions as a supervisor at the Baptist Memorial Hospital and as a Mental Health Nurse at various hospitals in San Antonio, Texas USA.
At Concord, Raymond’s primary duties include among others the following activities:
- Admit, screen and document client status and outcomes
- Obtain MD orders for CLASS Services
- Collaborate with other multidisciplinary health services for client welfare
- Create Nursing Assessments, Care Plans, and ICAPS for all admitted clients
- Admit clients to CLASS / HHSC Program for the State of Texas
